気管再建と下咽頭食道再建 -安全に行うための工夫-

腫瘍切除後の気管および上部消化管の再建は、頭頸部外科において依然として難易度の高い課題である。本発表では、異なる臨床状況に対して行っている二つの再建術式について述べる。
甲状腺癌の気管浸潤や気管原発癌により頸部気管が窓状に切除された症例に対しては、遊離前腕皮弁と遊離骨移植を組み合わせた二期的再建を行っている。前腕皮弁によって気管欠損部を被覆し、骨移植を支持組織として用いることで、気道の狭窄や虚脱を防止する。初回手術では気管皮膚瘻を残すことにより、術後の呼吸管理、皮下気腫の予防、遊離皮弁の血行モニタリングを容易にし、初回手術から半年ほど経過した後に気管皮膚瘻を閉鎖する。本術式は、安全性に配慮しながら喉頭機能を温存できる点に特徴がある。
一方、下咽頭喉頭頸部食道摘出術に縦隔永久気管孔造設術を伴う症例では、広範な切除によって上部消化管欠損と前縦隔の死腔が生じる。このような症例に対し、複数の血管茎を有する長い遊離空腸を用いた一期的再建を行っている。本術式では、一つの遊離空腸によって上部消化管を再建するとともに前縦隔の死腔を充填できるため、追加の皮弁移植を必要としない。
これらの術式は、難易度の高い頸部気管再建および上部消化管再建に対する、実用的かつ有効な選択肢となり得る。

Reconstructive Strategies for Cervical Tracheal Defects and Extensive Hypopharyngeal- Esophageal Defects

■ Abstract
Reconstruction of the trachea and upper digestive tract following tumor resection remains a major challenge in head and neck surgery.
This presentation describes two independent reconstructive techniques for distinct clinical challenges in the head and neck.
For partial cervical tracheal defects, staged reconstruction using a free forearm flap combined with a free bone graft provides vascularized soft tissue coverage and rigid structural support to maintain airway patency while preserving laryngeal function.
For patients requiring pharyngoesophagectomy with anterior mediastinal tracheostomy, single-stage reconstruction using a free jejunal flap with multiple vascular pedicles enables upper digestive tract reconstruction and obliteration of the mediastinal dead space with a single flap, avoiding the need for additional flap transfer.
Both techniques have demonstrated reliable clinical performance and satisfactory functional outcomes.
These techniques provide practical and effective reconstructive options for tracheal and upper digestive tract reconstruction in challenging cases.
